The Rural AIDS Action Network (RAAN) assists persons living with, affected by and at risk for HIV/AIDS in rural Minnesota. RAAN offers medical case management, HIV counseling, testing and referral, education and training services statewide.

RAAN is seeking a Medical Case Manager to join our team of professionals providing services to people living with HIV/AIDS in Minnesota. This position will provide medical case management services to persons living with HIV/AIDS in the Mankato/Worthington area. This position will also provide community outreach, HIV testing/counseling and referral in assigned area. Applicants must have reliable transportation, travel in and around the state of Minnesota is required.
Candidates must have strong community networking, volunteer management, communication and problem-solving skills and must be a self-starter, able to manage time and production with off-site supervision.
The ideal candidate will have a bachelors degree or higher and 3 - 5 years experience working in the HIV/AIDS field, social services, public health, medical field or other related field bilingual skills a plus.
Primary Responsibilities:
Responsible for initial and ongoing needs assessment, education (risk reduction), care planning, support, and advocacy with people living with HIV who meet case management criteria.
Help meet the needs of persons living with HIV who meet case management criteria by accessing, utilizing and extending community social service and volunteer programming/opportunities.
Provide free HIV tests throughout Region.
Provide prevention education and risk reduction communication to the broader community.
